Click on the links below to download the forms you can use to gather signatures. The “General Letter” allows for signatures from more than one person on each form. Use this for friends and family. This is the most important form as we need as many signatures as possible.


The actual bill, HR 5429, is available for you to download so that you can read it for yourself as well as allow those signing in support to read. It is only 2 pages…..not like the Health Care bill that fills a room with paper.

The “Depositor Letter” allow more space for depositors to give additional information if you so desire as well as tell your story. This could be very useful to the Senators and their staff to help them get the support we need. This form should be filled out IN ADDITION to the first General Form as our goal is to get as many signatures as possible.
